Preferred providers network

We have agreed rates with all our network Medical Providers and therefore you should identify yourself by presenting your membership card, to any provider you will be using as a Universal Life client. Failure to do so may result in a higher than expected charge for your treatment which we may not be able to settle in full.

If you require in-patient or daycare treatment and it is received at one of the listed network Medical Providers then, after our pre-approval, we will be able to make payment directly to the network Medical Provider on your behalf.

Any hospitalization expenses at a non-network hospital should be settled by you and then submit a claim form to Universal Life. This case may result in a higher than expected charge which may not be able to settle fully. In such a case, you will have to pay the difference.
